Quiz module: Setting up a self-evaluation form

In addition to delivering graded quizzes in the QuizBox module, self-evaluation forms can be created and managed too. Self-evaluation forms are an ideal way of asking students to reflect on their performance, or help focus their attention on the objectives being measured. You can use them before and after work is submitted or assessed. Self-evaluations can also be used to gather information on students’ effort, beliefs about their ability, goals, motivation, and so on. Such information can provide teachers with a better understanding of why students perform as they do and helps students realise their own needs/next steps.

This tutorial will show you how to create self-evaluation forms in your ClassSpace pages. Before starting the tutorial, ensure that the QuizBox module is on your current page layout and activated (for guidance on this, read this FAQ).

Creating a new self-evaluation form

  1. Click on the module "Options" link, and select the "Create a New Quiz" option.

  2. In the resulting pop-up window, select the "Create Self Evaluation" tab. You will be prompted to enter some details for the form before creating it.


    Begin by giving the form a title, and then a start and end time (i.e. when the form will be available to be completed by the students).


    Once you have entered the details required, click the "Create Evaluation Form" button to proceed.

  3. Next, you will be taken to the Edit Form window, which has four tabs along the top.


    • The "Summary" tab provides you with overall statistics on student completion rate.

    • The "Questions" tab is where you create the evaluation form itself and add/edit/delete questions.

    • The "Student Answers" tab is where you can bring up your class list and see their individual responses (only populated when a form is published).

    • The "Quiz Settings" tab allows you to modify the initial parameters set when created.

Editing the Form Instructions

Self-evaluation forms can be set with specific instructions such as guidelines, tips or any other information that may be required before they start. A default set of instructions is inserted into every new form, which you can see on the "Summary" tab under the "Instructions for students" section.


To edit this:

  1. Click on the "Edit Quiz Summary" button at the bottom.

  2. In the resulting pop-up editor, make the necessary edits, then click "Update Quiz Summary" at the bottom to save your changes.

Adding questions to the evaluation form

When you are ready to construct the evaluation form itself:

  1. Navigate to the "Questions" tab, then click on the "Add new Question" towards the top-right.


  2. In the Edit Question panel that appears, begin by selecting the type of question you want to create from the options along the top.


    • "Choice with one answer" means a multi-choice question where only one response is selectable.

    • "Choice with one or more answers" means a multi-choice question where several responses can be selected.

    • "Short answer question" allows the students to reply in an open text field.

  3. Next, in the "Question" field, type in the question you wish to pose to the students. This is a text-only field, so if you wish to add imagery or other media, see the section below on adding media to questions.

  4. Next, if either of the multi-choice question types is selected, you will see the option to "Load Template", which are a series of preset answer types for typical forms. The standard templates available are:

      • "1 - 6" - This is a simple numerical sequence (1, 2, 3, 4, 5, 6), useful for questions asking students to grade or rank their response in relation to a scale.

      • "Excellence - Cause for concern" - This is for reflective type questions, such as when reviewing own performance (Excellence, Very Good, Acceptable, Needs to Improve, Cause for concern).

      • "Agree - Disagree" - This is a variation on the numerical scale (Strongly agree, Agree, I don't know, Disagree, Strong disagree).

      • "Yes - No" - This provide a simple either-or option (Yes, No).

    Alternatively, you can click the "Clear" option and enter your own answer options in the fields provided. Each of these feature a text-only field, as well as the option to add a media file to the answer option (e.g. an image).


  5. Once you have entered the details required, click the "Save" button at the bottom to store the question and repeat the process to add more questions.

Adding/removing media to a question or answer

It is possible to attach an image, video or audio file to any question and/or answer in the evaluation form. To insert a media object into the form:

  1. Navigate to the "Questions" tab in the quiz, then click on the "Edit" link for the question to be modified.

  2. Towards the left of the question or answer, you will see the default "No Media" icon. Click on it to bring up the Media Browser.


  3. To upload a file to the Media Browser, click on the "Add Media" link on the top-right. Follow the instructions to select and upload a supported image, video, audio or ZIP file.

  4. Once uploaded, your Media Browser will display all the files available. Browse for the one you want to use, then click the "Insert" button.


To remove media from a particular question/answer:

  1. Click on the media thumbnail image for that question/answer.

  2. In the Media Browser window that pops up, click on the "Clear" button on the bottom.

To delete an uploaded media object from the quiz's storage:

  1. Click on the media thumbnail image to open the Media Browser window.

  2. Search for and select the media file to be deleted from the browser. Once the preview loads up, click the "Delete" button beneath it to permanently remove the file from the quiz's storage.

Publishing the evaluation form

Once you have finished setting up the form with the relevant instructions, questions and duration parameters, the last step is to publish it. Without publishing, the form will remain in a 'draft' state within the QuizBox—but no students will be able to see it. Publishing essentially makes the form available for the students to open and submit answers.

To publish the form, go to the "Questions" tab of the Self-Evaluation Form, then click the "Publish to students" button on the top-right.
